Enroll in the Supplemental Life Insurance Plan

To enroll:

Select the amount of your supplemental (optional) life insurance.

1. Select the Coverage level based upon your salary (from 1 up to a maximum of 5 times your salary).
2. Click Save to save your changes
   OR
   Click Cancel or Return to Overview to cancel what you've entered.

To cancel your participation:

1. Click in the box next to Cancel Supplemental Insurance coverage.
2. Click Save to cancel your participation
   OR
   Click * Cancel or Return to Overview* to cancel what you've entered.

Result: You return to the "Change Benefit Elections: Overview" page.
