FSA Review Your Name and Home Address

This screen shows your current name and home information. Review the information and if it is correct, click Continue. If you want to update the information, enter the changes at the appropriate places.
Note: Fields with the asterisk next to them are required fields.

Official Name

To change your official name, use the Name Change Notification Form.

MIT Contact Information

E-mail Address: Enter your e-mail address in the format xxx@xxx.xxx.
Office Phone: Enter your office phone number in the format 999-999-9999.
Site: Select the site location of your MIT office.
Building: Enter the MIT building where your office is located. You can use the Search button to find it.
Room: Enter the room number of your office.

Home Address and Phone

Country: Select the country of residence.
Address Line 1: Enter your street address.
Address Line 2: Enter additional address information such as P.O. Box number.
City: Enter the city. If the country is not US or Canada, enter the region (prefecture, territory, etc.).
State/Province: Select the state (US) or province (Canada).
Zip/Postal Code: Enter the zip or postal code.
Phone: Enter your home telephone number in the format: 999-999-9999 (for US or Canada). For other countries, any format will be accepted.

When you have entered all your changes, click Continue.

If you want to return to Benefits, click Home.
