Select a Provider for yourself and-or family members

Note: Select a primary care provider (PCP) for yourself and each family member enrolled in a medical plan.

If you need to find a provider, use Provider Search (listed on the right on the Employee Self Service page):

1. Select your medical provider from the dropdown list. The provider's network web site opens up in a second browser window.
2. Follow the web site instructions to search for a provider.
3. Write down your provider name and provider ID number.
4. Close the provider's network browser window and continue with the instructions below.

If you know the physician's name and provider ID number:

1. Type in the "Provider Name".
2. Type in the "Provider ID number" (required).
3. Click Save to save your entries
   OR
   Click Cancel or Return to Overview to cancel what you've entered.

Result: You return to the "Select a Provider for Yourself and Family Members: Overview" page.

Repeat PCP selection and enter Provider Name and Provider ID number for each dependent.
